110-300-0480(2) The provider stated they offer transportation to families. Licensor reviewed the required equipment and documentation and observed (a)the health history, emergency information and emergency medical authorization forms for children missing. (c) Licensor observed the provider does not have a complete first-aid kit. (f) Licensor observed the provider did not have a system for attendance that assures all children are accounted for each time children begin and end travel and every time children enter and exit the vehicle. 110-300-0480(3) Licensor reviewed the required equipment for transportation and observed the provider is missing (f)emergency reflective triangles or other devices to alert drivers of an emergency.
